Founded in 2005, Delta Waves, Inc constantly strives to be the local leader in providing sleep disorders and clinical research services. The Delta Waves team is committed to achieving and maintaining this goal by compassionate delivery of patient care, and regard for the highest ethics in business practices.

Why us?

If you, or a loved one, suffers from the effects of a sleep disorder and would like to be seen by our Sleep Specialist or take part in a sleep study, appropriate documentation is necessary. For a sleep study it is necessary for your physician to complete a sleep services order form (please see link below). Completed forms may be faxed to our scheduling department at 719-262-9285, along with your current insurance information & most recent clinic notes.
