For a sleep study it is necessary for your physician to complete a sleep services order form (please see link below).  Completed forms may be faxed to our scheduling department at 719-262-9285, along with your current insurance information & most recent clinic notes.

We do accept self –referrals for office consultations with our Sleep Specialist.

Once we have received the appropriate documentation, a member of our scheduling department will give you a call in order to schedule your appointment.
