Delta Waves accepts referrals from physicians, PAs or NPs from a variety of medical specialties. We offer a couple of referral options, allowing our providers to choose which process best suits the clinical needs of their patient.

Sleep Specialist Consultation Referral:

Physicians can refer patients directly to our sleep specialist for a thorough clinical evaluation of their sleep-related complaints. Following the sleep consultation, the patient will be scheduled for a sleep study if necessary.

Physician Direct Referral:

Delta Waves accepts direct referrals without requiring patients to schedule a sleep consultation visit with our sleep specialist prior to their sleep study.

Sleep Services Order Form:

Please include a copy of the patient’s history and physical information, most recent clinical notes, and insurance information. If your patient has had a previous sleep study at another facility, please include those results, as well.
